I need more support. I want my old form tutor from my old school

I want to be in contact with my old form tutor from my old school, she was the best form tutor who I have ever had. I miss her and I need to talk to her during this difficult first anniversary. I need help on how to contact her as she doesn't go to my old high school anymore. I need her help to help me through the anniversary.
